Job summary

Suvida Healthcare seeks a Care Coordinator to deliver high-quality care to our patients. You will work with the Nurse Care Manager to conduct daily follow-ups, manage DME/home health orders, triage calls, and retrieve hospital records for recently admitted patients.

You will perform clinical clerical tasks, support chronic and transition of care programs, and collaborate closely with the multidisciplinary team to ensure patient-centered care and improved outcomes.

Qualifications

• Minimum 3 years' experience as a Medical Assistant or LPN / LVN.
• 5 years' experience as Medical Assistant preferred.
• Bilingual in English and Spanish required.

Responsibilities

• Conduct daily patient follow-ups for high-risk patients.
• Process DME and home health orders.
• Support inbound triage calls and coordinate responses to acute needs.
• Retrieve hospital records for recently admitted patients.
• Perform clinical clerical tasks to support Chronic Care and Transition of Care programs.
• Collaborate with the Nurse Care Manager to ensure seamless patient care.
