Job summary

Devoted Health is seeking a Care Coordinator to support TOC Clinical Operations with post-discharge planning and follow-up. You will engage patients inpatient and after discharge, coordinating telemedicine visits, testing, and documentation while collaborating with clinicians to ensure smooth transitions of care.

The role requires strong organizational skills, healthcare experience, and bilingual proficiency in English and Haitian Creole.

Qualifications

• 2+ years of patient facing and care coordination experience within healthcare, managed care, home health, and/or community health setting.
• Experience working in a telemedicine environment.
• Fully bilingual (English/Haitian Creole).

Responsibilities

• Reach out to patients while inpatient to introduce TOC program and communicate post discharge follow up from TOC clinicians.
• Schedule post discharge follow ups (PCP/Specialty).
• Reach out to hospitals and provider offices to obtain clinical documentation.
• Submit Home Care and DME orders as well as vendor follow up.
• Coordinate lab testing and imaging, and track results to ensure review by ordering clinician.
