Job summary

Legacy Community Health is hiring a Bilingual Care Team Assistant in Houston. You will be the front-line contact for patients, assisting with calls, triage, and coordinating care with nurses and providers.

The role emphasizes compassionate, patient-centered support, documentation in EHRS, and collaboration with the care team to ensure smooth transitions and timely information delivery.

Qualifications

• High-school level education or equivalency.
• Medical Assistant Certification or its equivalent, or at least 2 years clinical experience.
• Must maintain BLS/CPR.
• Spanish fluency preferred.
• Experience with EHRS systems.
• Strong communication and customer service skills.
• Ability to multi-task in a fast-paced environment.
• Excellent organizational skills and discretion with confidential information.

Responsibilities

• Communicate with patients by phone and in person; route inquiries to providers and relay results as directed.
• Support pre-visit planning by reviewing schedules and preparing reports.
• Coordinate care transitions and facilitate referrals and access to ancillary services.
• Assist with medication refills and prior authorizations.
• Prepare forms for review and finalize with provider signatures; address urgent requests.
