Job summary

Legacy Community Health is seeking a bilingual Care Team Assistant (CTA) to serve as a trusted contact for patients, families, caregivers, and the care team. The role supports clinical coordination, patient communication, and clerical tasks to keep care connected and accessible.

You will assist with lab results, medication management, referrals, and pre-visit planning while helping ensure a compassionate, patient-centered experience in a busy clinic.

Qualifications

• High-school diploma or equivalent.
• Medical Assistant Certification or its equivalent, or at least 2 years clinical experience.
• Must maintain BLS/CPR.

Responsibilities

• Answer patient calls and triage using protocol-based parameters.
• Document in EHRS and route inquiries to providers.
• Return patient calls as directed by the provider.
• Communicate lab results and other information to patients promptly.
• Assist with pre-visit planning and care transitions as needed.
• Coordinate referrals and complete required paperwork.
• Support medication refills and prior authorizations as directed.
