Job summary

Legacy Community Health in Houston is seeking a Care Team Assistant (Bilingual Spanish). You will coordinate patient care, support lab results, manage referrals, and assist with medication-related tasks.

Primary duties include patient communication, charting in EHRS, and ensuring smooth care transitions within a PCMH framework. Responsibilities require bilingual Spanish proficiency, strong organizational and communication skills, and experience in ambulatory settings.

Qualifications

• High-school level education or equivalency.
• Medical Assistant Certification or 2 years clinical experience.
• Must maintain BLS/CPR certification.

Responsibilities

• Patient communication via call center/front desk and protocol-based triage.
• Document in EHRS; return patient calls and notify providers as directed.
• Coordinate care transitions, lab results, and referrals with timely updates.
• Assist with interpretation services and participate in daily huddles.
• Support medication management and prior authorizations as needed.
• Ensure patient privacy and maintain accurate, complete records.
