Job summary

Legacy Community Health is seeking a Bilingual Care Team Assistant to serve as a trusted contact for patients, families, and caregivers. You will support clinical coordination, navigate referrals, and assist with medication management and documentation in the EHRS.

Responsibilities include coordinating care, communicating with providers, and helping patients complete pre-visit planning and forms in a fast-paced clinic environment.

Qualifications

• High-school level education or equivalency.
• Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting.
• Must maintain BLS/CPR

Responsibilities

• Address patient calls via the call center and/or front desk using protocol-based triage parameters.
• Route inquiries requiring provider review to the responsible provider in the EHRS.
• Return patient calls as directed by the provider.
• Send results-related letters and deliver them to patients in a timely manner.
• Assist with transitions of care and ensure timely documentation and follow-up.
