Soni Family Practice and The Doctors Center is seeking a Care Navigation Coordinator to ensure seamless transition of care and excellent patient experience in the Health Services division. You will coordinate patient onboarding, care coordination, and referral coordination, and perform health risk assessments with new patients.

Responsibilities include scheduling visits, creating charts, verifying eligibility and benefits, coordinating referrals, and maintaining accurate EHR records.

Formación

• Must be detail-oriented and able to multi-task
• Excellent documentation and communication skills
• Knowledge of medical terminology and coding optional

Responsabilidades

• Schedule and coordinate patients' first office visits and ensure a positive experience
• Create patient charts and ensure documentation is complete
• Verify patient eligibility and benefits
• Coordinate referrals to specialists, hospitals, and other providers
• Verify insurance coverage and obtain pre-authorizations as needed
• Maintain accurate patient records in the EHR system
• Conduct health risk assessment calls to establish risk levels and care needs
• Communicate effectively with patients, families, providers, and staff
