Soni Family Practice and The Doctors Center in Lakeland, FL is seeking a Care Navigation Coordinator to ensure seamless transitions of care and an excellent patient experience.

You will onboard new patients, coordinate care and referrals, perform health risk assessments, and develop personalized care plans with the care team. Strong organizational skills and effective communication are essential for success.

Qualifications

• Must be detail-oriented and able to multi-task while solving problems.
• Excellent phone etiquette and ability to navigate computer systems proficiently.
• Team-oriented with professional interaction with patients, colleagues, physicians and staff.
• Strong organizational skills with the ability to prioritize and complete tasks on time.
• Knowledge of medical terminology, CPT, HCPCS, and ICD coding preferred.
• Bilingual is a plus.

Responsibilities

• Schedule first office visits and coordinate patient experiences.
• Create patient charts and ensure complete documentation.
• Verify patient eligibility and benefits.
• Request hospital records and other medical information.
• Schedule appointments with care coordinators and primary care physicians.
• Coordinate referrals to specialists, hospitals and other providers.
• Verify insurance coverage and obtain pre-authorizations as needed.
• Maintain accurate and timely documentation in the EHR system.
• Conduct health risk assessments (HRA) calls with new patients to establish risk levels.
• Collaborate with care coordinators to develop personalized care plans for new patients.
• Maintain up-to-date patient records and communicate with patients, families, providers and staff.
• Participate in quality improvement initiatives and adhere to regulatory requirements.
