• Analyze data to inform care decisions and drive outcomes.
• Lead or participate in care planning meetings and conferences.
• Educate patients and families on care plans and self-management strategies.
• Identify barriers to care and escalate issues as needed.
• Stay updated on regulations and best practices in disease management.
• Mentor junior care coordinators and contribute to quality initiatives.
