Job summary

Tri-County Care is seeking a Care Manager to work with individuals with intellectual/developmental disabilities and their families. You will identify goals, build plans, and help access Social Security, SNAP, Waiver programs, and other supports.

The role supports individuals of all ages, with periodic in-person visits required. A valid driver’s license is needed for travel reimbursement, and a quiet home workspace is preferred.

Qualifications

• A bachelor’s degree with two years of relevant experience or equivalent.
• RN license with two years of relevant experience or similar.
• Master’s degree with one year of relevant experience.

Responsibilities

• Collaborate with individuals, families and providers to set goals.
• Develop plans and assist with applications for supports and services.
• Coordinate care activities and advocate for person-centered outcomes.
