Job summary

The City of Lincoln is seeking a Care Manager to collaborate with individuals with intellectual/developmental disabilities, their families, OPWDD, and providers to help people live safe, fulfilling lives. You will identify goals, build plans, and assist with accessing supports and services to reach person-centered outcomes.

The role is remote with periodic in-person visits to Manhattan, NY, and surrounding areas. A valid driver's license is required for travel and reimbursement.

Qualifications

• Bachelor’s degree with two years of relevant experience.
• RN license with two years of relevant experience.
• Master’s degree with one year of relevant experience.
• MSC Service Coordinators grandfathered for continuity.

Responsibilities

• Collaborate with individuals and families to identify goals.
• Assist with applying to supports and services.
• Coordinate travel for in-person visits and care planning.
