Job summary

City of Lincoln is hiring a Care Manager to work with individuals with intellectual/developmental disabilities, families, and providers to support person-centered outcomes. You will help identify goals, build plans, and assist with applications for supports, services, and benefits.

The role requires a 4-year degree, sets expectations for travel to site visits, and offers a remote work arrangement with periodic in-person travel across the NY area.

Qualifications

• Bachelor’s degree with two years of relevant experience or RN license with two years of relevant experience.
• Master’s degree with one year of relevant experience; MSC grandfathering applies for continuity.
• Proven ability to communicate clearly and collaborate with teams and families.

Responsibilities

• Collaborate with individuals with disabilities, families, OPWDD and providers to support person-centered outcomes.
• Identify goals, build plans, and assist with applying to supports and services.
• Advocate for individuals in education, healthcare, and benefits processes.
• Travel for in-person visits as required; work remotely with potential regional office support.
