Job summary

Heritage Health Network is seeking a Lead Care Manager to support members through our Enhanced Care Management (ECM) program in San Diego County. This hybrid role combines field visits with remote planning to deliver personalized care and coordinated services.

You will serve as the primary contact, develop care plans, collaborate with healthcare teams, and address gaps in care while documenting interactions. Travel within Orange County may be required, and bilingual English/Spanish is preferred.

Qualifications

• Experience in care or case management (community-based preferred).
• Knowledge of Medi-Cal and ECM strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify and address gaps in care.
• Maintain accurate, timely documentation.
