Job summary

Titanium Healthcare is seeking a Lead Care Manager to oversee case management for ECM participants. The role blends field visits in San Bernardino and Riverside County with remote work, requiring strong coordination with doctors, housing navigators, and care teams.

The ideal candidate is energetic, self-starting, and capable of cross-functional collaboration to improve member health and well-being. Travel will be required to serve members effectively.

Qualifications

• 1+ year experience in healthcare, social services, or care coordination
• Current BLS certification from the American Heart Association
• Driver’s license with clean driving record and reliable transportation
• Diploma/GED required, associate degree preferred

Responsibilities

• Manage an assigned caseload of adult and pediatric members
• Conduct monthly in-person visits and home/community assessments
• Develop individualized care plans based on assessments and medical history
• Coordinate communication among healthcare providers, social workers, and care team
• Ensure documentation is accurate and compliant with policy and standards
• Advocate for patients to access services and navigate the healthcare system
• Provide member education on health topics and self-care strategies
