Job summary

The Inova Juniper program seeks a dedicated Care Coordinator III for a full-time day shift (Monday–Friday, 8:00 a.m.–5:00 p.m.). The role provides clinical coordination and facilitation to meet patients’ healthcare needs as they enter the system, working in a team-focused, fast-paced environment with strong communication across in-person, phone, and electronic channels.

Qualifications

• High School diploma or equivalent.
• 2 years of experience with call center, customer service or related profession.
• Experience in healthcare (preferred).

Responsibilities

• Answering and mitigating calls and providing intake services on Specialty Lines.
• Route patient to the appropriate sub-specialty service at client facilities after interpreting conditions.
• Coordinate with physicians, nurses, and staff to ensure smooth admission processes.
• Read and interpret multiple documents requesting services.
• Interact with client facility staff by telephone to intake calls.
• Maintain multiple databases related to the service line.
• Input calls for service in CAD and patient information into EHR.
• Document activities on shift using spreadsheets and email formats.
• Notify management of problems, concerns, and compliments in real time.
• Generate performance reports using computer software.
