Job summary

Inova Health System's Juniper Clinic is seeking a dedicated Registered Nurse Case Manager to join the Case Management Team. This Full-Time Day Shift opportunity runs Monday to Friday with 8:00am-4:30pm or 8:30am-5:00pm.

The role supports HIV/AIDS care, primary medical care, mental health and community resources, contributing to Inova's mission of safe, high-quality patient care in Northern Virginia.

Qualifications

• 1 year of case management and/or clinical care experience
• Bachelor’s Degree in Nursing or Associate’s Degree; if RN has an Associate’s Degree, plan to complete BSN within 5 years and meet with nurse leader as required

Responsibilities

• Provides discharge planning and continuity of care for acute and post-acute patients
• Collaborates with interdisciplinary team to coordinate discharge planning, referrals and follow-up
• Monitors and reports performance indicators and variances from plans of care
• Advocates for patients to resolve barriers to care progression and ensures safe, effective care
