Job summary

Family Choice Management Services in Westminster, CA seeks a Bilingual Patient Engagement Coordinator (English–Spanish) to drive outreach, care coordination, and education across Medi-Cal, Medicare, and Commercial populations. Collaborate with PCPs, IPA, and quality teams to close care gaps and improve preventive and chronic care management.

The role requires 1–2 years in patient engagement, bilingual English–Spanish, strong communication, and EHR proficiency.

Qualifications

• Bilingual fluency in English and Spanish (written and spoken).
• 1–2 years in patient engagement or care coordination preferred.
• Experience with Medi-Cal/Medicare or managed care is a plus.
• Strong communication and organizational skills.
• Proficiency with EHRs and care management platforms.

Responsibilities

• Outreach to patients via phone, email, SMS; engage in English–Spanish. (outbound/inbound)
• Coordinate with PCP offices to schedule preventive and chronic care visits; confirm completion
• Educate patients on preventive services and chronic disease management
• Motivate patients to adhere to care plans and relay concerns to care teams
• Collaborate with IPA nurses, coders and QI staff to support improvement projects and reporting
