Optum CA in Irvine is seeking a Patient Outreach Coordinator to join our team and support population health initiatives. You will perform proactive patient engagement, schedule preventive services, close care gaps, and assist with HEDIS, STAR, and Risk Adjustment projects in a fast-paced environment.

The role requires empathy, organization, and the ability to navigate healthcare systems, with full-time hours 8am–5pm PST, Monday–Friday. Training will occur during standard hours.

Formación

• High School Diploma or equivalent is required.
• 6+ months of Customer Service experience using phones and computers.
• Bilingual fluency in English and Spanish or Vietnamese.

Responsabilidades

• Conduct outbound calls, text messages, and other patient outreach activities to address preventive care and chronic condition care gaps.
• Assist patients with scheduling appointments, screenings, and follow-up visits with providers.
• Educate patients on preventive health services and chronic disease management.
• Support outreach campaigns related to HEDIS, STAR, and Risk Adjustment initiatives.
• Maintain outreach tracking records and document interactions in designated systems.
