Qualifications

• Community based experience with ECM populations: homelessness, at-risk or frequent hospital/ER users, serious mental health or substance use.
• Bilingual capacity preferred and bilingual Spanish fluency required.
• Must have a valid California driver’s license and a personal vehicle for local travel.

Responsibilities

• Outreach and engage potential members to provide information on ECM program offerings, eligibility, and enrollment.
• Travel between client locations to support outreach efforts and services.
• Maintain documentation with strong computer and typing skills and collaborate with care teams.
