Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs. The role coordinates with Care Teams and a psychiatric consultant, supports referrals, and delivers evidence-based treatments.

The position may include remote work days based on program metrics and performance. Requires LISW/LPCC/LMFT licensure and US work authorization, with a 40-hour week and full-time status.

Qualifications

• Master's degree in mental health counseling/healing arts from an accredited school.
• Highest clinical level license or local equivalent.
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for the Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants and telehealth partners.
• Maintain relationships with primary care teams and communicate care plan changes.
• Document encounters and counseling sessions in the electronic medical record.
• Participate in regional and organizational efforts to advance the program.
