Job summary

Oak Street Health is seeking a Behavioral Health Specialist in the United States to support care management and provide short-term, evidence-based psychotherapy for patients with behavioral health needs.

The role involves coordinating with primary care teams and a psychiatric consultant, with potential remote work days based on program metrics and performance. Requires advanced licensure and Spanish language ability as preferred.

Qualifications

• Master’s Degree in the field of mental health counseling/healing arts from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for common mental health and/or substance use disorders.
• Experience with assessment and treatment planning for common mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and provide or coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with the psychiatric consultant, including warm handoffs for telehealth visits.
• Develop relationships with primary care teams and communicate changes in care plans.
• Accurate and timely documentation of patient encounters in the EMR.
• Participate in regional and organizational efforts to advance the program.
• Possibly covering multiple clinics depending on panel sizes.
