Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs. The role collaborates with Care Teams and a psychiatric consultant, with potential remote work days based on program metrics.

Responsibilities include screening, care planning, referrals, Counseling, and warm handoffs for telehealth. Requires MSW/LCPC/LMFT, highest license, and US work authorization.

Qualifications

• Master’s Degree in mental health counseling or related field from an accredited program.
• Highest clinical license or local equivalent required.
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health needs.
• Familiarity with brief, structured intervention techniques (e.g., MI, CBT).
• U.S. work authorization is required.

Responsibilities

• Collaborate with primary care teams to refer patients to behavioral health program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate with psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants; facilitate telehealth warm handoffs.
• Maintain accurate documentation in the electronic medical record.
• Engage in regional/organizational efforts to advance the program.
• Possibly cover multiple clinics depending on panel sizes.
• Perform other duties as assigned.
