Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management services and short-term psychotherapy across primary care clinics. You will coordinate care with a psychiatric consultant, assess patient needs, and make referrals to behavioral health resources.

A remote work day may be available based on metrics. The role requires a master's in a mental health field, the highest clinical license, and experience with screening and treatment planning.

Qualifications

• Master's degree in mental health counseling or related field.
• Highest clinical license (or local equivalent).
• Experience with screening for mental health/substance use disorders.
• Experience with assessment and treatment planning for behavioral health issues.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, CBT).
• US work authorization required.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to Oak Street Health Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and arrange referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with the psychiatric consultant and telehealth activities.
• Build relationships with primary care teams and communicate changes in care plans.
• Document patient encounters accurately in the EMR.
• Participate in regional and organizational efforts to advance the program.
• May cover multiple clinics depending on panel sizes and maturity.
• Other duties as assigned.
