Oak Street Health is seeking a Behavioral Health Specialist to join our primary care care teams. You will provide care management and short‑term, evidence‑based psychotherapy to patients with behavioral health needs, coordinating with a psychiatric consultant and external resources.

Responsibilities include screening, assessment, care planning, referrals, and counseling, plus coordination of telehealth handoffs with care teams. Remote work days may be possible based on metrics.

Qualifications

• Master's degree in a mental health field from an accredited program.
• Licensed or highest clinical level required.
• Experience with screening for mental health or substance use disorders.
• Experience with assessment and treatment planning for common mental health conditions.

Responsibilities

• Collaborate with primary care teams to identify patients for the Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and arrange referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs.
• Maintain accurate documentation of encounters and sessions.
• Participate in regional efforts to advance the Behavioral Health Program.
