Zusammenfassung

Oak Street Health seeks a Behavioral Health Specialist to deliver care management and short-term psychotherapy for patients with behavioral health needs. You will coordinate with Care Teams across regional clinics, assess patient needs, develop care plans, and refer to the psychiatric consultant and external resources.

A remote-work option may be available based on program metrics. Requires LISW/LPCC/LMFT, a master's degree in a mental health field, and US work authorization.

Qualifikationen

• Master's degree in a mental health field from an accredited school.
• Highest clinical level license (or local equivalent).
• Experience with screening for common mental health and/or substance use disorders.
• Experience with assessment and treatment planning for common mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• US work authorization.

Aufgaben

• Collaborate with primary care teams to identify patients for referral to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and refer to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including warm handoffs for telehealth.
• Maintain relationships with primary care teams and communicate changes in care plans.
• Document encounters and counseling sessions in the EMR.
• Participate in regional/organizational efforts to advance the program.
• Cover multiple clinics as needed.
• Other duties as assigned.
