Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term evidence-based psychotherapy to patients with behavioral health needs. The role coordinates with Care Teams across multiple clinics and may involve remote work days based on performance.

Requirements include a Master’s in mental health counseling, licensure at the highest clinical level, experience with screening and assessment, and proficiency in brief interventions.

Qualifications

• Master's Degree in mental health counseling/healing arts from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to Oak Street's Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and provide or facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth and care plan collaboration.
• Develop relationships with primary care teams and communicate changes in care plans.
• Accurate and timely documentation of patient encounters in the EMR.
• Participate in regional efforts to advance the Behavioral Health Program.
• Possibly cover multiple clinics depending on panel sizes.
• Other duties as assigned.
