Job summary

CVS Health and Oak Street Health invite applications for a Behavioral Health Specialist. This full-time role provides care management and short-term psychotherapy to patients with behavioral health needs, collaborating with Care Teams and a psychiatric consultant.

The position may allow one remote work day based on metrics and program success. Qualified candidates hold a Master’s in a mental health discipline, licensure at the highest level, and US work authorization.

Qualifications

• Master's degree in mental health counseling or related field from an accredited program.
• Highest clinical license level or local equivalent.
• Experience screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for these conditions.
• Familiarity with brief, structured interventions (e.g., M.I., Behavioral Activation, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to refer patients to the Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth.
• Build relationships with primary care teams and communicate plan changes.
• Document patient encounters accurately in the EMR.
• Participate in regional efforts to advance the Behavioral Health Program.
• Possibly cover multiple clinics as needed.
• Other duties as assigned.
