Qualifications

• Bachelor’s degree with at least 1 year of relevant experience.
• “Associates degree” with a minimum of 2 years in a related field.
• High school diploma with 3 years of related experience may be considered.
• Certified Behavioral Health Technician preferred.
• Strong communication and time management skills.
• Ability to work within an interdisciplinary team.
• CPR/First Aid certification and Arizona fingerprint clearance.
• Experience with EMR systems (AdvanceMD preferred).
• P4P compensation structure familiarity.

Responsibilities

• Provide ongoing case management to patients including coordinating behavioral health and medical services.
• Use motivational interviewing to engage patients and promote health behavior changes.
• Identify community resources to assist patient access to services.
• Collaborate with patients and families to implement care plans with the clinical team.
• Document accurately in EMR and healthcare databases.
