Sanitas in the Phoenix area seeks a Case Management Coordinator to manage transitions of care for members after discharge and for those with chronic conditions. You will coordinate follow-up care, create personalized plans, and educate patients and caregivers to prevent readmissions.

The role requires clinical knowledge, strong communication, and proficiency with EMR systems; bilingual English/Spanish is preferred. Travel up to 50% and hybrid work arrangement are included.

Qualifications

• High school diploma or equivalent.
• Graduation from a nationally accredited Medical Assistant program.
• Active MA national certification.
• CPR/BLS for Healthcare Providers required.
• Advanced English; bilingual Spanish/Creole preferred.

Responsibilities

• Conduct timely post-discharge outreach (within 24-72 hours) to assess member needs and identify gaps in care.
• Coordinate follow-up appointments and post-acute services with providers and home health agencies.
• Develop individualized care plans based on program categories and risk factors.
• Educate members using motivational interviewing to empower self-management.
• Document interactions and interventions in EMR/care management platforms accurately and timely.
