Job summary

Horizon Health and Wellness in Queen Creek, AZ is seeking a full-time Care Manager to coordinate behavioral health services for individual clients. You will advocate for clients, develop care plans, and connect them with the appropriate community and clinical resources to ensure timely, coordinated care.

Ideal candidates have a high school diploma or GED with at least 4 years in behavioral health, strong organizational skills, and the ability to work evenings as needed.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience.
• One year case management experience strongly preferred.
• Ability to create plans and summaries based on client needs.
• Certification in first aid, CPR and Crisis De-escalation (training provided if needed).
• Must be at least 21 years old with valid driver’s license and reliable transport.
• Prior behavioral health experience preferred and ability to coordinate with clinical team.

Responsibilities

• Coordinate clients' services in a timely, coordinated manner.
• Advocate for clients and assist with care planning and referrals.
• Maintain communication with the clinical team and community partners.
• Support time-management and documentation for care plans and summaries.
• Ensure safety and compliance during service delivery.
