Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Confident working at home with virtual collaboration.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.

Responsabilités

• Assess referred members’ needs and eligibility and determine approach to case resolution.
• Coordinate care with providers, programs, and community resources.
• Apply clinical guidelines and standardized case management plans.
• Monitor plans of care and document outcomes with the care team.
