Job summary

CVS Health in Illinois is seeking a dedicated Case Manager to coordinate benefits and health services for members across Medicare/Medicaid, leveraging clinical models and community partnerships.

You will assess needs, plan care, advocate, and collaborate with providers while traveling within Southloop and Chinatown areas as required; a master's degree in behavioral health and licensure are preferred. This role is a full-time, 40-hour-per-week position with a comprehensive benefits package.

Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Travel up to 50-75% in Southloop, Chinatown and surrounding areas.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.
• Efficient computer skills including navigating multiple systems.
