Job summary

COORDINATED CARE service inc is seeking a Behavioral Health Community Services Coordinator to coordinate community-based behavioral health services, including referral management, intake coordination, service assignment, data tracking, reporting, and communication with referral sources. You will maintain a caseload and provide rehabilitative and recovery-oriented services per NYS requirements and agency policies.

Services may include Health Home, HARP, HCBS, CORE or other programs.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Behavioral Health, or related field preferred.
• 1–3 years of experience with substance use disorders, serious mental illness, or co‑occurring disorders required.
• NY state driver license with acceptable driving record required.
• CRPA, CPS, CASAC or similar credential preferred where applicable.
• Experience coordinating referrals and managing caseloads in an interdisciplinary setting.

Responsibilities

• Coordinate referrals into community-based behavioral health services with timely communication.
• Monitor referral status, authorizations, service thresholds, and client engagement.
• Assign referrals and coordinate intake activities with program leadership.
• Maintain program data, tracking systems, reports, and performance metrics.
• Maintain an assigned caseload and provide services per approvals.
• Complete assessments, service plans, crisis plans, and discharge planning.
• Collaborate with Health Home Care Managers, managed care, providers, and partners.
• Participate in interdisciplinary treatment teams and case conferences.
• Provide rehabilitation, recovery, skill-building, advocacy, and supportive services.
• Conduct community-based visits, including home visits, as clinically appropriate.
• Assist clients in accessing medical, behavioral health, vocational, housing, and other resources.
• Provide transportation or coordinate it when appropriate.
• Respond to crisis situations and follow agency protocols for safety.
• Maintain compliance with documentation standards and regulations.
• Participate in staff meetings, supervision, trainings, and quality improvement activities.
• Assist with onboarding and training of new staff.
• Maintain knowledge of NYS behavioral health regulations and program requirements.
• Demonstrate professionalism, cultural competency, and person-centered care.
• Perform other duties as assigned.
