Job summary

Lighthouse Behavioral Wellness Centers in Ardmore, OK seeks a full-time Behavioral Health Case Manager. You will provide individualized case management, care planning, and crisis stabilization to clients with mental illness and substance use issues, coordinating with families and providers to support recovery.

You will complete assessments and documentation, travel up to 25% for community-based services, and participate in team meetings and training to maintain high-quality care.

Responsibilities

• Provide varied case management, rehabilitative and wellness services to promote recovery.
• Schedule and facilitate wraparound family team meetings with formal and informal supports.
• Complete SNCD assessments, care plans, and safety plans as required.
• Enter data into required systems per ODMHSAS timelines.
• Provide crisis management and stabilization as assigned.
• Assist clients to engage in outpatient care and overcome barriers.

60 college credit hours + 12 months behavioral health experience

High school diploma + 36 months direct mental illness/substance use experience
