HOPE Family Health seeks a Community Health Worker to support Behavioral Health Telephone services, helping patients schedule, follow up, and coordinate care.

You will communicate with patients and care teams, identify barriers to care, and connect clients with HOPE programs and community resources. This role emphasizes bilingual language access where qualified and alignment with HOPE policies.

Formación

• High school diploma or equivalent required; relevant experience in health care or care coordination preferred.
• Ability to communicate clearly and respectfully with patients, colleagues, and community partners.
• Ability to use electronic health records and standard office software; knowledge of local resources preferred.
• Additional language proficiency or interpreter qualifications may be required for some assignments.

Responsabilidades

• Build respectful, trusting relationships with patients and families; communicate in a culturally responsive manner and support patient choice.
• Contact patients by telephone, electronic message, or in person; relay approved information and route clinical concerns appropriately.
• Help patients understand appointments, care plans, referrals, tests, and next steps using team-approved information.
• Identify barriers to care and connect patients with HOPE programs and community resources; follow up as assigned.
• Coordinate with medical and behavioral health teams and external partners to support continuity of care.
• Maintain timely records of contacts, referrals, and services in the health record; protect confidentiality.
