Qualifications

• Bachelor’s degree in social service field preferred; or 5 years behavioral health experience with 1 year RBHA/AHCCCS may replace degree
• High school diploma or GED required
• Minimum 1 year of experience with State AHCCCS and RBHA clients; preferred
• Excellent customer service skills
• Excellent computer skills
• Knowledge of Arizona Department of Health Services and RBHA requirements
• Knowledge of chronic disease management
• Ability to complete PRAPARE assessments
• Knowledge of community resources
• Thorough knowledge of current versions of DSM and ICD
• Ability to complete Behavioral Health Assessment with clinical oversight
• Bilingual English/Spanish strongly preferred
• Valid Arizona driver’s license
• Valid Level One Fingerprint Clearance Card issued by Arizona DPS

Responsibilities

• Participate in screening of new service requests within the center according to policies and procedures
• Participate in regular interdisciplinary staff meetings
• Develop treatment/service plans with clients and establish effective alliances
• Collaborate with medical team to utilize behavioral health services
• Research potential patients for the Behavioral Health Consultants using AZARA
• Respond to positive screenings, triage scores, and refer to BHC as needed
• Provide direct client services and assist with resources for behavioral health needs
• Arrange transportation for patients for medical or behavioral health appointments
• Track clients and generate reports to monitor quality of care
• Document all service contacts promptly and accurately
• Support location with tasks when Integrated Care Coordinator is unavailable
• Disperse resources to clients based on presenting needs
• Distribute mail to patients after follow-up contact attempts
• Make reminder calls for follow-up appointments with BHC
• Collaborate across departments on integrated behavioral health
• Disseminate educational information relevant to integrated behavioral health
• Perform other duties as assigned consistent with Adelante Health Care policies
