Job summary

The Care Coordinator is a member of Cumberland River Behavioral Health in Alabama, working within a multidisciplinary team to provide services to adults, children and families. Qualifications

• Bachelor’s degree in Psychology, Sociology, Human Services or Nursing, or a Master’s Degree in Human Services field.
• Healthcare customer service experience preferred.
• Must pass criminal background check and drug screening.

Responsibilities

• Conduct initial screenings and referrals to psychiatric, medical, social, educational and other support services.
• Identify client needs and assist in engaging community resources to promote wellness and recovery.
• Respond to emergency situations promptly.
• Maintain communication with referring providers, especially state hospital system.
• Provide warm hand-off when clients are referred to partnering providers.
• Provide crisis assistance to the consumer and coordinate needed services.
• Work with a multidisciplinary team and independently as needed.
• Maintain documentation of services, referrals, and follow-up with clients and providers.
• Other duties may be assigned.
