Job summary

Heritage Behavioral Health Center in Decatur, Illinois is seeking a dedicated Care Coordinator to ensure seamless, ongoing support for individuals accessing behavioral health services in their program area.

The role tracks clients from initial engagement through connections to recommended treatments, collaborating with primary care and community resources to ensure timely follow-up and adherence to treatment plans, improving client outcomes.

Qualifications

• High School Diploma or equivalent with 5 years of supervised mental health or social service experience.
• Bachelor’s degree in social work, psychology, education, or human service field preferred.
• Minimum of 21 years of age and valid driver’s license with reliable transportation.

Responsibilities

• Ensure clients are connected to recommended services including behavioral health treatment, primary care, specialty care, and community-based supports.
• Proactively follow up with clients to monitor progress, identify barriers to care, and provide ongoing support.
• Coordinate appointments with primary care teams, behavioral health providers, and community partners to ensure adherence to treatment recommendations.
• Educate clients and families on the importance of care continuity, self-advocacy, and available resources.
• Assist clients in navigating healthcare and social service systems, addressing obstacles to care access.
• Participate in interdisciplinary team meetings to discuss client progress and ensure comprehensive service delivery.
• Advocate for client needs and promote self-determination and empowerment.
