Job summary

The Care Coordinator at Cumberland River Behavioral Health is a key member of a multidisciplinary team serving adults, children, and families in Kentucky. You will help clients access medical, psychiatric, social, and other support services, coordinate referrals, and respond to crises.

Qualifications

• Bachelor’s, Master’s, or Associate degree as specified and pass background check and drug screening.
• Healthcare customer service experience preferred.

Responsibilities

• Conduct initial screenings and referrals to psychiatric, medical, social, educational and other support services.
• Identify client needs and engage community resources to promote wellness and recovery.
• Respond to emergency situations in a timely fashion.
• Maintain ongoing communication with referring providers, particularly state hospital system.
• Provide warm hand off when clients are referred to partnering providers.
• Provide crisis assistance and coordinate needed services.
• Work with multidisciplinary team and independently when necessary.
• Maintain documentation of services, referrals, and follow ups.
