Job summary

The position requires a bachelor’s or master’s degree in related fields, strong communication, and crisis management skills. Travel and flexible scheduling may be involved to meet agency needs.

Qualifications

• Bachelor’s degree in Psychology, Sociology, Human Services or Nursing or a Master’s Degree in Human Services.
• Associate’s Degree in Nursing qualifies in specific cases.
• Experience in healthcare customer service is preferred.

Responsibilities

• Conduct initial screenings and referrals to psychiatric, medical, social, educational and other support services.
• Identify client needs and engage community resources to promote wellness and recovery.
• Respond to emergency situations in a timely fashion.
• Maintain communication with referring providers, especially the state hospital system.
• Provide warm hand-offs when clients are referred to partnering providers.
• Provide crisis assistance to consumers and coordinate needed services.
• Demonstrate ability to work with a multidisciplinary team and independently.
• Maintain documentation of services, referrals and follow-ups.
• Other duties may be assigned.
