Job summary

NovumHealth is hiring a Community Care Coordinator to help members navigate behavioral health and substance use services within Salt Lake County, UT. You will coordinate care plans across providers and advocate for patients to improve long-term outcomes.

The role requires 1+ year in care coordination or related fields, a healthcare degree preferred, and willingness to travel to multiple sites. Onsite work only; competitive benefits offered.

Qualifications

• 1+ year of experience in case management, care coordination, behavioral health, or substance use services.
• Bachelor’s degree in healthcare, social work, nursing, or related field preferred.
• CPR/First Aid certification or willingness to obtain.
• Must pass drug screening and background check to start employment.
• Must have independent transportation to visit multiple locations.

Responsibilities

• Assess healthcare needs and identify gaps in care within the community.
• Collaborate with providers, social service agencies, and organizations to develop care plans.
• Assist patients in navigating the healthcare system and accessing services.
• Schedule and coordinate appointments across therapy, psychiatry, MAT, and recovery programs.
• Facilitate communication between patients, providers, and other stakeholders.
• Monitor treatment adherence and provide motivational support for recovery milestones.
• Conduct home or community visits to assess living conditions and support systems.
• Advocate for members’ needs across healthcare, housing, and social systems.
• Document interactions and care plans in electronic records.
