Stormont Vail HealthCare, Inc. is seeking a Behavioral Health Admissions & Nurse Navigator to coordinate patient access and navigation across the behavioral health continuum of care.

This part-time weekend role works with patients, families, providers and internal teams to ensure timely admissions and smooth transitions between inpatient and outpatient services. The position requires a Master’s degree in psychology, social work, counseling or a related field, with RN licensure or other eligible

Qualifications

• Master's degree in psychology, social work, counseling, or related field; RN licensure accepted as substitute.
• 1 year psychiatric or acute care experience required; behavioral health setting preferred.
• Strong psychiatric assessment skills and crisis management能力.
• Excellent communication, problem-solving, and collaboration across teams.

Responsibilities

• Lead and coordinate behavioral health admission processes and one-call/one-number access.
• Conduct comprehensive psychiatric assessments across age groups to determine level of care.
• Facilitate admissions to inpatient and outpatient services based on criteria.
• Coordinate alternative placements when admission is not appropriate or capacity is limited.
• Identify and address barriers to admission and patient placement.
• Document intake and admission data accurately in the electronic health record; ensure regulatory compliance.
