Job summary

Haven Behavioral Healthcare, part of Oceans Healthcare, seeks an Outpatient Navigator to facilitate inpatient transitions into the Oceans Outpatient Program following discharge and in line with regulatory standards. You will coordinate admissions, discharge planning, and family involvement while ensuring smooth patient transitions.

Bachelor's degree in a related field and at least 1 year of psychiatric patient experience are preferred, with a focus on clear communication and culturally sensitive

Qualifications

• Bachelor’s degree in related field or equivalent combination of education and related work experience.
• At least 1 year’s experience working with psychiatric patients preferred.

Responsibilities

• Educate inpatient groups about IOP services and benefits.
• Facilitate the admit process for patients entering the IOP program and complete required documentation.
• Ensure new admit documentation is provided to Outpatient Coordinator on time.
• Meet and escort patients at discharge to the IOP group.
• Coordinate with families for involvement in therapy sessions.
