Job summary

Behavioral Health Network (BHN) is seeking an Access Navigator to join the CBHC Access Center/Central Intake team. You will welcome individuals seeking behavioral health services, collect information, guide them through the navigation workflow, and coordinate with clinicians and community providers to ensure timely access to care.

This onsite role emphasizes customer service, confidentiality, and teamwork within a long-standing Western Massachusetts organization.

Qualifications

• High school diploma or GED required; associate degree preferred.
• 1–2 years of experience in behavioral health, care management, human services, or related field.
• Knowledge of mental health conditions, substance use, care management, and related healthcare systems.
• Ability to access behavioral health, healthcare, social-service, and other supports.
• Strong customer-service, interpersonal, advocacy, and written communication skills.
• Willingness to work onsite at the CBHC Access Center location.

Responsibilities

• Welcome individuals seeking behavioral health services and provide timely assistance, referrals, and support in person, by phone, or via telehealth.
• Serve as primary contact for assigned individuals, guiding them from inquiry to connection with services.
• Collect and verify demographic, insurance, and other information for electronic records.
• Navigate the CBHC Access Navigation workflow to assess needs and inform referrals.
• Use screening tools and information to identify treatment needs and appropriate BHN/community services.
• Collaborate with clinicians and care teams to respond to immediate needs.
• Coordinate with BHN programs and community providers to connect individuals with services.
• Track progress and follow up on outstanding needs; address barriers to engagement.
• Maintain confidential, organized documentation and records.
• Participate in meetings and training; support other Access Navigators as needed.
