Job summary

Behavioral Health Network (BHN) is seeking an Access Navigator to join the CBHC Access Center/Central Intake team. You will welcome clients, gather necessary data, and guide individuals through intake, triage, and referrals across BHN and community services.

You will work with master’s-level clinicians and care teams to identify needs, coordinate resources, and monitor progress, ensuring timely access to behavioral health care.

Qualifications

• High school diploma or GED required; associate degree preferred.
• 1–2 years of experience in behavioral health, care management, human services, or a related field required.
• Knowledge of mental health conditions, substance use, care management, and healthcare systems.
• Proficiency with computers, electronic records, word-processing, email, and other communication systems.

Responsibilities

• Welcome individuals seeking behavioral health services and provide timely assistance and referrals.
• Serve as the primary contact, maintaining communication from initial inquiry through connection to services and resources.
• Confirm and collect demographic, insurance, and relevant information for the electronic health record.
• Guide individuals through the CBHC Access Navigation workflow to inform targeted care and referrals.
• Use screening tools to identify potential treatment needs and appropriate BHN and community services.
• Provide assigned triage coverage and collaborate with master’s-level clinicians and the integrated behavioral health team.
• Coordinate with BHN programs, care management teams, and community providers to connect services efficiently.
• Track progress and follow up on outstanding needs, addressing barriers to engagement or enrollment.
• Maintain organized, accurate, confidential documentation and records.
• Participate in team meetings and required training, supporting fellow Access Navigators.
