Job summary

ADP, INC. is seeking a Behavioral Health Medical Navigator to provide targeted case management and healthcare navigation for individuals with mental health needs. The role collaborates with the clinical team to identify navigation needs and coordinates access to medical and behavioral health services.

This position supplements the client’s PP/Case Manager and aims to reduce barriers to care, improve continuity, and strengthen coordination across services within the contractor’s scope.

Qualifications

• Bachelor’s degree or equivalent in a helping field.
• Experience in behavioral health, case management or healthcare navigation.
• Knowledge of community resources and healthcare systems.
• Experience coordinating referrals for behavioral health needs.
• Strong organizational, communication and documentation skills.
• Ability to manage referrals independently and collaborate with teams.
• Ability to protect client confidentiality and PHI.

Responsibilities

• Review assessments and treatment plans to identify navigation and case management needs.
• Collaborate with the PP/Case Manager and treatment team to set priorities.
• Assist clients in navigating hospitals, clinics and behavioral health providers.
• Coordinate access to medical, psychiatric and specialty-care appointments.
• Facilitate referrals to providers and community resources.
• Provide follow-up after hospitalization or significant care events.
• Identify barriers to access and follow-through on care.
• Assist with telehealth access, patient portals and scheduling.
• Support referrals within the contractor’s scope and coordinate communication.
• Maintain professional boundaries and protect client information.
