Responsibilities

• Elicits patient information from referring MD and outside facility nurse for triage.
• Uses triage tools and algorithms with patient access services to transfer patients timely.
• Acts as liaison to ensure continuity of care for patient and family.
• Assesses patients and families before initial consult including intake and history.
• Collaborates with providers and staff to facilitate complete care.
• Documents visit reason, tests, referrals, and current medications.
