Qualifications

• Bachelor's Degree in Nursing required.
• Registered Nursing License in Massachusetts required.
• Clinical nursing experience 2-3 years required.
• Patient flow or bed access experience 1-2 years preferred.
• Ability to interact effectively with intra- and outside-hospital physicians and staff.

Responsibilities

• Elicits patient information from referring providers to triage and place appropriately.
• Uses triage tools and algorithms to transfer patients in a timely, clinically appropriate timeframe.
• Acts as a liaison to ensure continuity of care for patient and family; supports financial navigator.
• Performs initial nursing assessment including intake, diagnosis, history, symptoms, psychosocial and educational needs.
• Coordinates with providers and staff to facilitate comprehensive patient care and decision-making.
• Documents visit reason, tests ordered, referrals, and current medications.
