Interim HealthCare of Oklahoma City is seeking an experienced Home Health RN Case Manager in Oklahoma City. You will play a hands-on role in developing and adjusting the plan of care with LPNs, therapists, and physicians, focusing on the full patient picture.

Qualifications

• Active RN license in Oklahoma.
• Home health experience preferred.
• CPR certification.

Responsibilities

• Develop and adjust the plan of care with the care team.
• Complete visits and OASIS assessments.
• Coordinate with LPNs, therapists and physicians to ensure full patient care.
• Educate patients and families on diagnosis, medications and home safety.
• Advocate for patients to drive real outcomes.
