Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager to lead the plan of care, work with LPNs, therapists, and physicians, and ensure comprehensive patient assessment and documentation. This role emphasizes autonomy and patient relationships, not just charting.

Qualifications

• Active RN license in Oklahoma.
• Home health experience preferred, but other clinical backgrounds with strong assessment skills are welcome.
• CPR certification required.

Responsibilities

• Develop and adjust the plan of care for patients with the care team.
• Complete visits, OASIS assessments, and thorough documentation.
• Coordinate with LPNs, therapists, and physicians to prevent gaps in care.
• Educate patients and families on diagnosis, medications, and home safety.
• Advocate for patients to drive meaningful outcomes.
